JOB SUMMARY:

Responsible and accountable for providing continuum of care for cancer patients who are diagnosed and/or are to receive cancer treatment. This role will include guiding newly diagnosed patients/families through the complex array of options, advocacy, responding to questions, assisting in clinical and support group referrals and scheduling, care coordination and enhancing the patient-physician relationship.

Essential Job Functions:

  • Manage the lung cancer screening program by working closely with primary care physicians for patientreferrals who meet eligibility criteria.
  • Participate in weekly Tumor Board meetings by submitting prospective cases (newly diagnosed) andfollowing up on multidisciplinary recommendations.
  • Conduct and/or coordinate the hospital's navigation process for all cancer sites including psychosocialdistress screening and Survivorship Care Plan discussions. Report all activities annually to the CancerCommittee.
  • Assist with various cancer program activities including Community Needs Assessment, follow up of all andrecent cancer patients in the registry, quality studies, prevention and screening community outreach.
  • Must effectively communicate (verbal and written), work in a team environment and perform independentlywithin the scope of practice

* Other duties may be assigned based on departmental needs and within the RN scope of practice.

Qualifications

MINIMUM QUALIFICATIONS:

  • Current Registered Nurse (RN) license in the State of Texas required.
  • BSN required ; MSN preferred
  • BLS certification
  • At least 1-2 years experience in oncology nursing; National Certification in Oncology preferred
  • Oncology Nurse Navigator Certification required or obtained within 1 year
  • Bilingual (English/Spanish) preferred.
