Position: Manager Case Management

Job Duties:

  • Organizing patient care by prioritizing, delegating, delivering and evaluating on and ongoing basis.
  • Engage critical thinking skills in making decisions regarding patient care including knowledge and integration of relevant standards resources and data
  • Utilize the knowledge base of nursing education and experience to deliver nursing care using the nursing process according to the hospital policy procedures and protocols
  • Collaborate with the nursing team to facilitate overall improvement quality of care patient experience and cost reduction
  • Collaborate with the physician team to facilitate overall improvement in quality of care and patient experience
  • Advocate or ethical and holistic care promoting the autonomy dignity and health of patients from diverse ethnic and social backgrounds
  • Coordinate with external agencies to provide a continuum of care for the pediatric patient and their family
  • Navigate and collaborate with utilization management team to ensure proper billing and reimbursement
  • Monitor patient progress and provide education and support to patients and their families to ensure they receive the best possible care tailored to their specific needs
  • Create comprehensive individual care plans based on the patient's need including medical treatment medication management therapy services and community support
  • Identify and excess appropriate community resources like transportation housing assistance and support groups
  • Responsible for day-to-day leadership activities managing a team of clinical professionals and the execution of exemplary cross continuum care coordination
  • Lead and attend daily patient progression rounds to ensure appropriate resources are being used to care for patients
  • Lead and attend weekly long stay patient meeting with a multidisciplinary group of providers to ensure that patients who are hospitalized over 7 days are appropriately resourced while they are admitted and when they are being prepared for discharge
  • Lead, evaluate, set goals and objectives, monitor performance and provide feedback to the pediatric care managers
  • Clearly communicate expectations policies and update the team actively listening to the employees concerns and feedback
  • Identify and address conflicts facilitate productive discussions and reach solutions

Requirements:

  • Bachelor's Degree in Nursing (or foreign equivalent)
  • Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)
  • 2 years of clinical experience as a registered nurse
  • 2 years leadership experience in progressive management role as a charge nurse

Location: Tacoma, WA

For this specific position, the salary offered will be no less than $174,436.28 per year. The pay scale for similar roles is $154,939.20-$213,969.60 per year.
