Position: Manager Case Management Job Duties: Organizing patient care by prioritizing, delegating, delivering and evaluating on and ongoing basis. Engage critical thinking skills in making decisions regarding patient care including knowledge and integration of relevant standards resources and data Utilize the knowledge base of nursing education and experience to deliver nursing care using the nursing process according to the hospital policy procedures and protocols Collaborate with the nursing team to facilitate overall improvement quality of care patient experience and cost reduction Collaborate with the physician team to facilitate overall improvement in quality of care and patient experience Advocate or ethical and holistic care promoting the autonomy dignity and health of patients from diverse ethnic and social backgrounds Coordinate with external agencies to provide a continuum of care for the pediatric patient and their family Navigate and collaborate with utilization management team to ensure proper billing and reimbursement Monitor patient progress and provide education and support to patients and their families to ensure they receive the best possible care tailored to their specific needs Create comprehensive individual care plans based on the patient's need including medical treatment medication management therapy services and community support Identify and excess appropriate community resources like transportation housing assistance and support groups Responsible for day-to-day leadership activities managing a team of clinical professionals and the execution of exemplary cross continuum care coordination Lead and attend daily patient progression rounds to ensure appropriate resources are being used to care for patients Lead and attend weekly long stay patient meeting with a multidisciplinary group of providers to ensure that patients who are hospitalized over 7 days are appropriately resourced while they are admitted and when they are being prepared for discharge Lead, evaluate, set goals and objectives, monitor performance and provide feedback to the pediatric care managers Clearly communicate expectations policies and update the team actively listening to the employees concerns and feedback Identify and address conflicts facilitate productive discussions and reach solutions Requirements: Bachelor's Degree in Nursing (or foreign equivalent) Current Registered Nurse license in Washington State or Multistate License endorsement (MLS) 2 years of clinical experience as a registered nurse 2 years leadership experience in progressive management role as a charge nurse Location: Tacoma, WA For this specific position, the salary offered will be no less than $174,436.28 per year. Pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
