POSITION SUMMARY

The Clinical Social Worker provides essential psychosocial support to patients, families, and the interdisciplinary care team by identifying social, emotional, and environmental factors that may affect health outcomes, treatment compliance, and discharge planning. This role requires advanced clinical judgment and a broad range of therapeutic skills to deliver high-level interventions in a complex healthcare setting. The Clinical Social Worker serves as a vital member of the care team, providing compassionate, evidence-based psychosocial services in alignment with JHACH policies and procedures.


KEY ACCOUNTABILITIES

  • Conduct comprehensive psychosocial assessments to identify patient and family needs that may impact medical treatment, coping, and overall outcomes
  • Perform suicide risk assessments and, when indicated, initiate Baker Act procedures and coordinate placement in an appropriate psychiatric facility
  • Facilitate referrals to multidisciplinary team members and connect patients and families with community-based resources to support care coordination and reduce psychosocial distress
  • Deliver therapeutic interventions including crisis intervention, grief and supportive counseling, protective services, problem-solving support, strengths-based interventions, coping skills development, and psychoeducation
  • Collaborate with the medical team to develop individualized care plans and address psychosocial barriers affecting discharge readiness
  • Maintain accurate, timely electronic documentation, data collection, and recordkeeping for all patient interactions and interventions
  • Communicate effectively with interdisciplinary team members regarding patient needs, interventions, and progress, and actively participate in team meetings, huddles, and care rounds


QUALIFICATIONS

  • Master of Social Work (MSW) degree from an accredited program
  • Minimum of 2 years of clinical social work experience
  • Current Florida Licensed Clinical Social Worker (LCSW) licensure
  • Strong listening, verbal, and written communication skills


Salary Range: Minimum 30.64/hour - Maximum 49.02/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
