Inova Fair Oaks Hospital is looking for a dedicated Social Worker Case Manager to join the Case Management Team. This role will be Part-Time Day shift, 24 hours per week, Monday - Friday day shift schedule with rotating weekends and holidays.

Social Worker Case Manager Job Responsibilities:

  • The Social Worker Case Manager 1 evaluates the ability of patients to progress throughout the continuum of care.
  • Works collaboratively in communication with physicians, nursing and other members of the multidisciplinary care team to effect timely and appropriate patient management.
  • Showcases a working knowledge in utilization management, managed care and payer issues.
  • Provides discharge planning and continuity of care for assigned patients in the acute and post-acute setting, with an understanding of pre/post-acute resources.
  • Provides coordination of services and acts as a key Liaison between patients, families and the interdisciplinary healthcare members.

Minimum qualifications:

  • Masters Degree in Social Work
  • One - two years of experience in clinical care or clinical case management
  • Basic Life Support through the American Heart Association upon start

Social Worker Case Manager 1 Preferred Requirements:

  • Current or recent case management experience
