The Nurse Coordinator plays a vital role in the Lung Transplant Program(s), offering direct care to patients with complex medical conditions. They serve as the primary point of contact, coordinating pre-transplant evaluations, managing post-transplant follow-up care, and providing patient education. This position requires collaboration with a diverse team, including physicians and support staff, to deliver exceptional care and manage the unique needs of transplant patients.

PRE-TRANSPLANT ASSIGNMENT -

Scheduling of patient evaluations, patient education, and orientation to the transplant process. Monitoring of the evaluation process for patients, ensuring that the patient is being moved through the evaluation process in a timely manner. Acts as a liaison for multiple hospital departments during the patient evaluation process. Facilitate patient activities with consulting departments and physicians. Communicates effectively with peers and other members of the health care team, external contacts and uses appropriate channels of communication. Assessment of patient and family needs and communication of these needs to appropriate services, financial counseling, social work, nutrition etc. Provision of both inpatient and outpatient care for patients throughout all phases of the transplant process.

POST-TRANSPLANT ASSIGNMENT-

Coordinates all post-transplant patient care per physician orders and per protocols in the outpatient and clinic setting. Facilitate patient activities with consulting departments and physicians. Communicates effectively with peers and other members of the health care team, external contacts and uses appropriate channels of communication. Assessment of patient and family educational and other needs and direct communication of these needs to appropriate services, financial counseling, social work, nutrition etc. Provision of both inpatient and outpatient care for patients throughout all phases of the transplant process.

Facilitation and attendance at weekly heart or lung transplant patient selection meetings including documentation in TITUS/UNOS of all patients approved for wait listing and following up on all actions identified during the weekly meeting. Attends and participates in team meetings

Manages logistics of donor offers for Heart and/or Lung transplant patients during regular working hours. On occasion, may cover donor offers after hours, Holidays and weekends.

Coordinates all teaching for pre/post Heart or Lung transplant patients. Provides cross coverage to other pre/post Heart or Lung Transplant coordinators for vacations and sick leave

Assists with the maintenance and development of the Heart and Lung Database(s)

As appropriate, stays abreast of changes in Federal, State or Local Regulatory Agency requirements related to transplant services and reporting. Keeps a working knowledge of UCSF Policies and Procedures, follows guidelines appropriately and contributes to the maintenance of documents.

Obtains necessary information for all registries

Maintain communications with UNOS regarding patient listing, status changes, letters of appeal and requests for pre-transplant data and removal from the UNOS waitlist

Insurance authorization - assisting financial counselors in obtaining authorizations by providing patient information to insurance companies for certification of treatment modalities.

Assists with the collection and participation in all supported research protocols

Required qualifications:

  • Minimum of five (5) years of adult solid organ transplant nursing experience as an outpatient or inpatient Transplant Coordinator.
  • Five (5) years of RN experience in end-stage organ-specific disease, transplantation, and managing patients with organ failure.
  • Proven organizational skills and the ability to work independently, managing multiple concurrent tasks in a high-stress environment.
  • Excellent communication skills, both written and verbal, and the ability to conduct oneself professionally.
  • Computer literacy, including proficiency in MS Word, Outlook, internet use, and a willingness to learn and adapt to new EHR systems.
  • Strong MS Office skills, particularly in Word, Excel, and PowerPoint, and proficiency in using databases.
  • Active Registered Nurse (RN) licensure in the State of California, ensuring compliance with regulatory standards.
  • Active American Heart Association BLS certification, demonstrating commitment to patient safety and quality care.
  • Preferred qualifications include a BSN or MSN degree, as well as experience in assessing advanced heart failure/cardiac and/or pulmonary disease.
  • Clinical competency in managing transplant candidates and recipients, along with EPIC/APEX experience, is highly advantageous.

Preferred qualifications:

  • Working knowledge in assessing advanced heart failure/cardiac and/or pulmonary disease

  • Clinical competency in managing transplant candidates/recipients

License/Certifications:

  • Active Registered Nurse licensure in the State of California
  • Active American Heart Association CPR certification

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF.
