Summary:

This position supports clinical programs and Medicaid AE patients by providing care navigation for patients with a behavioral health, substance use disorder, or social determinant of health needs.

Essential Functions of the Position

  • Coordinates and tracks behavioral health referrals to internal and external behavioral health clinicians and resources.
  • Triages patients identified with social determinates of health (SDOH) or substance use disorder (SUD) needs to coordinate and track referrals to community-based organizations (CBO).
  • Maintains extensive knowledge of community-based organizations, external resource availability, and eligibility guidelines for each.
  • Facilitates transportation eligibility, scheduling, and other logistics for appropriate Medicaid AE patients.
  • Performs triage of notifications from CBOs and alerts appropriate clinical staff.
  • Collects and updates the release of information (ROI) documentation to ensure coordination of patient care between organizations.
  • Participates in interdisciplinary care conferences with primary care providers and other care management teams to develop an individualized plan of care.
  • Assists with clinical administrative support to Brown Health Medical Group Primary Care's Medicaid AE program and point of contact for community-based organizations.
  • Participates in Behavioral Health and AE team meetings.
  • Complies with federal and local confidentiality laws, including HIPAA, ensuring patient privacy.
  • Adheres to Brown Health Medical Group Primary Care's guidelines and policies for protecting patients' demographic, clinical and financial information.
  • Performs other job-related duties as assigned.

Education and Experience

  • Minimum of high school diploma or equivalent, Associates Degree preferred
  • Additional training in the medical field with a social service background or demonstrated possession of the competencies necessary to perform the work
  • Experience working in a clinical setting with a focus on behavioral health or social services

Knowledge, Skills, and Ability

  • Proficiency in the use of an EMR and knowledge of medical terminology
  • Ability to handle high volume and diverse assignments
  • Exercise sound judgment and decision-making. Able to assess and differentiate priorities
  • Excellent interpersonal skills and ability to work with and through others to achieve results
  • Excellent written and verbal communication skills
  • Must handle the most sensitive and confidential matters with the utmost discretion
  • Proficiency in use of various applications such as Microsoft Word, Excel, Web-based applications and EMR/EHR

Pay Range:

$20.96-$34.61
