Tabitha Hospice in Lincoln, NE is seeking a Hospice Registered Nurse to work as needed.

The Nurse Practitioner is responsible for the completion of face-to-face client visits for the purpose of determining continued eligibility criteria for hospice care in collaboration with the Hospice Medical Director.

Essential Functions

Provides client assessment and face to face visit for the purpose of establishing hospice eligibility criteria and documentation of criteria to ensure regulatory compliance.

Ensures the timely and adequate delivery of Hospice services to clients, including responsible for care plan management and oversight.

Provides for development of the initial plan of care, evaluates client for any immediate needs such as medications, durable medical equipment or supplies and arranges delivery.

Maintains clinical record documentation according to agency policy/procedures and regulations.

Actively participates as an Interdisciplinary team member, including offering insight and suggestions for care for all clients served by the team.

Contributes to continuity of care and effective communication between client/family, Hospice team members, Home Health Agencies, Inpatient Facilities, Ancillary Services, and Physicians.

Accepts responsibility of departmental and professional growth and development by evaluating delivery of care and serving as a role model.

Accepts responsibility for providing on-call, holiday and weekend nursing visits to Hospice clients, on an as needed basis during usual circumstances.

Supports and interprets Tabitha Hospice and departmental philosophy and objectives.

Maintains knowledge of and adheres to established organizational and departmental policies and procedures.

Assures delivery of optimum service by following assigned work schedule.

Participates in agency and community health programs to promote growth and understanding of hospice and home health services upon supervisor's request.

Assists with community and facility educational presentations and public relations tasks as assigned.

Ideal Candidate

AGE REQUIREMENT: At least age 18.

EDUCATION: Successful completion of a Nurse Practitioner (NP) or Physician's Assistant (PA) course of study and Master's Degree required.

EXPERIENCE: Two years of acute care, long term care, or hospice nursing experience within the last five years required.

LICENSURE: NP License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required. CPR Required. Valid Nebraska Driver's license (unless otherwise allowed by law) and a good driving record required.
