Job Title: Community Health Specialist

Job Summary: Are you passionate about making a positive impact in your community? We are seeking dedicated Community Health Specialists to join our team! In this non-clinical, hourly position, you will play a vital role in supporting health program activities, including health fairs, community events, and targeted outreach initiatives. You will provide essential screening services, teach health promotion classes, and connect participants with evidence-based programs to enhance health outcomes. This position reports directly to the Health Promotion Program Manager.

Major Job Responsibilities:



  • Education & Training: Deliver engaging and up-to-date curriculum for health promotion classes, including CPR and Mental Health First Aid.
  • Screening Services: Conduct screening activities by taking biometric measurements, implementing screening questionnaires, and referring eligible participants to relevant programs.
  • Community Referrals: Connect individuals with community-based programs and services, ensuring they access the support they need.
  • Record Keeping: Maintain accurate and up-to-date records on program participation and evaluations, adhering to Health Promotion protocols.
  • Resource Knowledge: Stay informed about available community resources to best serve the needs of participants.
  • Continuity of Care: Foster a smooth continuity of care from hospital settings to home and community environments.


Additional Responsibilities:



  • Demonstrate the hospital's core values and caring habits in all duties.
  • Attend mandatory departmental meetings and continuing education sessions to stay updated on hospital initiatives and best practices.
  • Maintain effective communication with your supervisor and co-workers to ensure collaboration and support.
  • Adhere to attendance and punctuality requirements to maximize your contribution to the organization.
  • Uphold the HCGH Mission Statement by prioritizing the needs of every patient.


Skills & Abilities Required:



  • Strong communication skills, both verbal and listening.
  • Proficient problem-solving abilities to address inquiries, complaints, and presentations for patients, families, staff, and community members.
  • Proficiency in Microsoft Word and familiarity with other computer systems and software.


Education & Experience:



  • High school diploma or GED is required, with an interest in behavioral, community, or public health.
  • Previous experience in a community setting is preferred.

Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
