Overview We are seeking a Population Health Management Consultant with specialized expertise in Behavioral Health to join our Business Analysis Department at eClinicalWorks. This role is critical in supporting our customers with the successful implementation of our Population Health Management (PHM) solutions, with a strong emphasis on behavioral health integration and care coordination. As part of our expanding PHM Consulting Team, you will serve as the subject matter expert for outpatient behavioral health management and 24-hour care settings, ensuring that our solutions meet the unique needs of behavioral health providers and organizations. Your insights will help shape workflows, optimize patient engagement strategies, and improve outcomes for populations requiring mental health and substance use disorder services. The PHM Consultant will also work with the customer to establish a project plan that aligns with their strategic goals, help customize the product, process, and workflows as well as train end-users to successfully use the eClinicalWorks PHM solutions for their requirements. Each consultant will have their own list of projects and will be responsible for making sure that their clients receive any assistance required to successfully participate/satisfy the requirements of the selected Population Health Management initiatives.
