Case Manager - Sage Ridge

AllHealth Network is currently looking for qualified experienced Case Managers to assist clients with complex issues and link clients with mental health resources to join our team at our new Sage Ridge location. We are looking for dedicated professionals passionate about behavioral health services. Job Description:

Case Manager



  • Meet with clients throughout their stay to collaboratively determine continuing care needs in a structured and simplified manner
  • As appropriate, consult with family members in identifying collateral information critical to removing potential gaps in treatment
  • Work to integrate support for clients in a comprehensive manner to address medical, social, and mental health needs
  • Establish and maintain effective working relationships with referents and family members through regular follow-up
  • Participate as an active member of the treatment team coordinating post discharge plans with the clinical and medical teams
  • Facilitate communication between clients and continuing care resources to ensure smooth transition to the next level of care
  • Participate in conflict resolution and crisis interventions with clients as needed
  • Facilitate transfer and discharge processes in a timely manner and in coordination with nursing and clinical teams.
  • Complete all necessary documentation and record keeping with accuracy and attention to detail
  • Adhere to confidentiality laws including 42CFR Part 2, HIPAA and HITECH
  • Follow all AllHealth Network policies and procedures
  • Complete all required trainings as listed in Relias Learning (both online training and face-to-face training) within required timelines
  • Perform other duties as required within the scope of the position and the experience, education and ability of the employee.


Qualifications:



  • Bachelor's degree in psychology, social work, or related field. Equivalent work experience may be substituted for education requirements
  • Certified Addiction Technician designation or NADAAC equivalent is preferred
  • CPR/First Aid certification
  • Previous work experience in mental health or substance use disorders
  • Experience in case management, care coordination, and/or community health required
  • Demonstrated knowledge and understanding of addiction and mental health issues


Skills/Knowledge:



  • Computer skills adequate for required tasks. Must be proficient in Microsoft Office
  • Ability to relate with detachment, understanding, and empathy to individuals with substance use disorders and their families
  • Ability to communicate effectively in written and verbal formats
  • Excellent motivational interviewing skills and an ability to establish a therapeutic alliance with clients
  • Skilled in identifying psychosocial stressors for clients to anticipate appropriate services
  • In-depth knowledge of pragmatic and immediately useful resources for clients
  • Skilled in presenting information in a culturally competent and non-stigmatizing manner
  • Personal maturity as reflected by sound judgment and decision-making
  • Ability to work as a positive and collaborative team member
  • Authentic commitment to AllHealth's mission and vision, actively working to further organizational objectives
  • Must have valid Colorado driver's license and acceptable motor vehicle record if driving AllHealth vehicle or driving for AllHealth business
  • If recovering from a substance use disorder, one year of continuous sobriety is required

Pay Rate:

$25 - $29 an hour

The base salary range represents the low and high end of the AllHealth Network salary range for this position. Actual salaries will vary and may be above or below the range based on various factors including but not limited to experience, education, training, merit, and the ability to embody the AllHealth Network mission and values. The range listed is just one component of AllHealth Networks' total compensation package for employees. Other rewards may include short-term and long-term incentives as well as a generous benefits package detailed below.
