Job Title: Social Worker

POSITION SUMMARY:
The Social Worker provides comprehensive psychological, social, and practical support to patients and families in collaboration with physicians, social workers, and the interdisciplinary team. This role includes conducting assessments, developing care plans, addressing psychosocial barriers to care, and connecting patients with resources to optimize health outcomes and safe transitions.



ESSENTIAL FUNCTIONS:




  • Perform psychosocial assessments and develop individualized care plans.



  • Deliver direct psychosocial support, including crisis intervention, grief counseling, abuse/neglect assessments, and patient/family education.



  • Collaborate with the interdisciplinary team to coordinate care, support safe discharge planning, and resolve barriers.



  • Maintain timely, clear documentation that meets departmental and regulatory standards.



  • Build and maintain community resource networks to assist patients with needed services.



  • Participate in family meetings and interdisciplinary rounds as needed.



  • Provide mentorship to students and entry-level social workers; may supervise if approved by the Maryland Board of Social Work Examiners.



  • May make home visits and provide coverage during absences, vacancies, or on-call shifts.



  • Participate in quality improvement initiatives and other duties as assigned.





QUALIFICATIONS:




  • Education: Master's degree in Social Work (MSW) required.



  • Licensure: Active Maryland LCSW-C required.



  • Experience & Skills:




    • Strong knowledge of community resources and the psychosocial impact of illness.



    • Ability to work independently in a healthcare setting.



    • Excellent assessment, counseling, and documentation skills.



    • Effective communication and conflict resolution skills.



    • Proficiency with electronic medical records and standard office technology.

Salary Range: Minimum $34.11/hour - Maximum $56.34/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.



In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
