Position Summary

The Pharmacy Clinical Coordinator acts as the principal point of contact for safe and appropriate medication use through education and physician liaison activities. Provides leadership to the department in the provision of clinical pharmacy services and drug information to medical, nursing, and ancillary staff. Facilitates the development, implementation, and maintenance of systems that promote optimal patient outcomes through safe, appropriate, and cost-effective medication therapy. The scope of this position may include inpatient and/or ambulatory pharmacy.

Principal Duties and Responsibilities

The following is a summary of the major functions of this individual's job

  • Develop, maintain, and monitor clinical pharmacy services through the continuum of care.

  • Develop, maintain, and supervise clinical training/education of pharmacists and support staff.

  • Ensure continued competence of clinical staff and provide input into clinical performance.

  • Provide education and training to medical, nursing, and ancillary staff regarding pharmacotherapy or related topics.

  • Actively participate as pharmacy representation on Pharmacy and Therapeutics and other committees as assigned and implement their decisions throughout the facility.

  • In conjunction with the Director of Pharmacy or Clinical Manager, coordinate the pharmacy's quality assurance program, including medication use evaluation, medication safety and stewardship initiatives.

  • Oversees activities of pharmacy residents and/or students as needed.

Minimum Education and Training

Graduate of an accredited College of Pharmacy.

Current Kentucky/Indiana Pharmacist License or eligible.

Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification (if applicable) or within 6 months of start date.

Completion of PGY-1 ASHP-accredited Pharmacy Residency preferred or minimum of 3 years of experience in hospital practice.

BCPS preferred.
