BASIC PURPOSE

The purpose of the Day Habilitation Specialist position is to help individuals develop and maintain skills for daily living, community participation and sustain self-sufficiency.



  1. ESSENTIAL DUTIES AND RESPONSIBILITIES
  2. Conduct intake and program eligibility for Housing Navigation Services
  3. Manage cases actively receiving Housing Navigation Services
  4. Conduct case management and provide service coordination
  5. Develop individual housing plans
  6. Provide education on tenant roles, rights, and responsibilities
  7. Provide transportation for housing & service navigation appointments
  8. Liaise between tenants and landlords to assist with issues/disputes
  9. Conduct landlord engagement
  10. Develop and maintain a housing inventory data list
  11. Conduct habitability inspections for potential housing units
  12. Coordinate medical referrals in coordination with contracted managed care plans
  13. Participate in weekly case conference meetings
  14. Participate in program and agency trainings as assigned
  15. Maintain accurate participant records in various information management systems; and generate reports as requested
  16. Maintain and execute confidential information according to HIPPA standards
  17. Maintain a highly detailed and organized filling system
  18. Ensure intake procedures utilize harm reduction and housing first principles
  19. Check and respond to emails and voicemails on a regular basis
  20. Adhere to confidentiality standards
  21. Other duties as assigned


KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS REQUIREMENTS



  1. Ability to speak and write the English language at a high and professional level
  2. High degree of confidentiality
  3. Able to endorse and promote The Salvation Army's mission
  4. Minimum two (2) years of social service/work and/or related experience preferred
  5. Computer literate, in Windows environment (Microsoft Office), Publisher, Excel and Outlook preferred
  6. Excellent communication skills, both written and verbal.
  7. Excellent and professional telephone etiquette and presence
  8. Excellent organizational skills
  9. Strong ability to utilize a high level of time management and handling multiple tasks



CERTIFICATES, LICENSES, REGISTRATIONS



  1. High school diploma
  2. Must possess a valid California Class C Driver License, and ability to drive a Salvation Army vehicle
  3. Must be 21 years or older
  4. Authorize The Salvation Army to add name to the CA DMV Pull Notice Program
  5. Complete The Salvation Army vehicle course training



PHYSICAL REQUIREMENTS:



  1. Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis
  2. Ability to grasp, push, and/or pull objects
  3. Ability to reach overhead
  4. Ability to operate telephone



  1. Ability to lift up to 25-40 lbs.



  1. Ability to operate a computer
  2. Ability to process written, visual, and/or verbal information
  3. Ability to operate basic office equipment and tools PC, Fax Machine, Telephone, Calculator, Copier, Printer.

Education
High School or Equivalent (preferred)
Experience
