The successful candidate:

This is a Full-time position where you will work Monday - Friday 8:00 AM to 4:00 PM.

At City of Hope, our phenomenal Occupational Therapists deliver individualized comprehensive care to patients through rehabilitation department utilizing therapeutic use of self, innovative technology and collaboration among patients. Is responsible and accountable for providing, delegating and guiding patient care to achieve an optimum level of function for each patient through treatment, education and direction of progressive exercise and activity.

Conducts educational in-service on benefits of occupational therapy to patients/families, other disciplines (hospital staff), and to the community. Non-patient related duties include, but is not limited to, assisting in clerical duties of the
department, posting charges, & light housekeeping duties within the Rehabilitation Department. Works under the general and direct supervision of the director of rehabilitation services department.

  • Reads and interpret consults to identify necessity and appropriateness of referral for assessment and evaluation of patients. Evaluate patients and develops a plan of care within 24 hours of referral. Administers occupational therapy interventions outline in the plan of care.

  • Documents evaluation results, treatment plans, patient response, and discharge summary according to AOTA and Joint Commission requirements- initial evaluation and progress notes. Send appropriate documentation (Outpatient initial evaluation and Prescription) to referring physician for signature. Address patients by their preferred name and address geriatric population by their last name unless indicated otherwise.

  • Teach patient/family caregiver the identified learning needs Document patient/ family/caregiver's understanding on the teaching form by end of shift.

  • Post charges on treatments rendered on patient accounts

  • Transport patients using department procedures as requested. Perform transfers and ADL training with appropriate assistive device/s

  • Clean department environment and equipment per department guidelines following infection control procedures.

  • Follows established City of Hope and department policies, procedures, objectives, performance improvement, attendance, safety, environmental, and infection control guidelines, including adherence to the workplace code of Conduct and Compliance Plan.

  • Practices a high level of integrity and honesty in maintaining confidentiality.

  • Performs other related duties as assigned or requested

Minimum Requirements

  • Must be a graduate from an AOTA approved school of Occupational therapy or a school approved by the World Congress of Occupational therapy and must have passed the national certification examination

  • Current Illinois state license in occupational therapy

  • CPR/BLS certification strongly preferred
