Responsibilities
  • Patient Assessment & Care Planning - Conduct initial and comprehensive patient assessments, develop individualized care plans in collaboration with physicians, and coordinate interdisciplinary services to ensure high-quality patient outcomes in a home health setting.
  • Direct Patient Care & Education - Deliver physical therapy treatments according to physician orders, monitor patient progress, and provide education to patients and caregivers on disease management, treatment plans, and home safety to promote independence and recovery.
  • Documentation & Compliance - Maintain accurate and timely clinical documentation, including OASIS assessments and care plan updates, while adhering to regulatory standards, agency policies, and quality assurance initiatives to optimize patient care and outcomes.

Qualifications

Education:

  • Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to physical therapist entry-level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association. DPT

    Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided.

Experience:

  • One year experience, including experience in performance of OASIS, in skilled home health care

License(s)/Certification(s):

  • Valid license as PT in state(s) where providing care.
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.

  • Mandatory Reporter: Child & Dependent Adult Abuse.

  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
