Overview:

The Critical Care Coordinator is an individual who supports the critical care service line by being an active member of the multidisciplinary team by coordinating with physicians, pharmacist, ancillary and case management departments and serving as a liaison and adjunct support for the critical patient regardless of physical location with a particular focus on time critical diagnosis such as sepsis. This navigator will correspond and collaborate between physician, nursing, and other health care providers across the continuum. The Critical Care Coordinator responds as assigned to patient care areas including but not limited to Emergency room, nursing floors, step down and critical care patient care areas. The navigator will ensure that patients receive timely, safe, effective and high quality care- will assist with triage and coordination of patients in the Emergency Department and on Medical, Surgical or Cardiac floors to provide critical care expertise and collaboration with intensivist. They will serve as a resource to nursing staff and physician staff in the Intensive Care Unit. They will coordinate processes and enhance collaboration and quality clinical outcomes as made evident by specified metrics, optimizing performance and adoption of evidence-based practice. These individuals must possess the qualities of a leader and be able to provide support, mentorship and guidance to patients and families as well as members of the healthcare team. He/She must be able to promote an environment conducive to learning for patients as well as other healthcare providers, display a positive presence with ability to adapt quickly in a changing environment. The Critical Care Coordinator may act as a scribe for the physician during a sepsis activation. A Critical Care Coordinator must perform duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

Qualifications:

  • Experience: At least one year of previous experience in critical care healthcare environment.
  • Required Education: Nursing Degree from an accredited school
  • Preferred Education: BSN
  • License: current RN license in our state
  • Certifications: Basic Life Support certification through the American Heart Association
  • Other: Must have solid foundation in clinical assessment skills, operational performance improvement, and general healthcare business operations. Strong analytical skills, effective problem-solving, organizational, and relationship skills and excellent interpersonal and communication skills
