Department Description

The UCSF Wellness Center for Youth with Chronic Conditions is an innovative new center that partners with patients, families, health care providers, and researchers to create a lifetime of wellness for children and adolescents living with chronic illness. Our center focuses on the co-creation of individualized pathways towards wellness for children with chronic illnesses and their families. We partner with patients over the long haul to provide personalized holistic support - connecting them with wellness services, providing support in the transition from pediatric to adult medical care, and helping them build a community of peers. By embracing a wellness-centered framework for care, we seek to compliment what is already available within the traditional medical system, and to address important gaps in empowerment and psychosocial connection.
